In vitro activity:
The effect of calcipotriol and betamethasone on the pro-inflammatory mediators IL-17C, IL-20, IL-6 and IL-8 released from keratinocytes (KCs) upon stimulation with Th17 cell-related cytokines (IL-17A, IL-22 and TNF-α) was evaluated (Fig. 5). Upon treatment with calcipotriol, betamethasone, or the combination, IL-17C and IL-20 secretion was found to be inhibited by 75%, 56% and 86% and by 82%, 60% and 90%, respectively (P < 0.0001 compared to vehicle) (Fig. 5, upper panel). IL-6 secretion was inhibited by 62%, 38% and 73% of the vehicle control with calcipotriol, betamethasone or the combination, respectively (P < 0.0001 compared to vehicle) (Fig. 5, lower panel). Treatment with calcipotriol or betamethasone alone inhibited the secretion of IL-8 by 44% of the vehicle control, while the combination inhibited the secretion of IL-8 more profoundly (64% inhibition) (Fig. 5, lower panel). In summary, these data suggest an additive anti-inflammatory effect in KCs by the two agents in combination in line with the findings from the DC and T cell cultures and the psoriasis explants. In vivo activity:
The potential of optical coherence tomography (OCT) to determine changes in thickness of the epidermis in response to the topically applied anti-psoriatic betamethasone dipropionate (BD) was evaluated. GsdmA3Dfl/+ mice have an inflammatory hair loss phenotype that includes hyperproliferation and epidermal thickening, hence a potential psoriasis model. Changes in epidermal thickness were evaluated over a period of 10 days, with the mice treated with BD. Initial baseline measurement revealed an average epidermal thickness of 26.92 ± 1.17 μm. After 10 days of treatment with BD, the average epidermal thickness was reduced by 38.8% (P = 0.0001).
