Rusfertide (PTG-300) is a hepcidin mimetic that regulates iron homeostasis by inhibiting ferroportin-mediated iron export from cells. In vitro studies demonstrate that rusfertide effectively reduces ferroportin expression on the surface of macrophages and enterocytes, leading to decreased iron efflux and intracellular iron accumulation. This action mimics endogenous hepcidin and results in a dose-dependent suppression of iron availability for erythropoiesis. Studies in primary human hepatocytes and cell-based models also show that rusfertide induces hepcidin-like activity without significantly altering inflammatory cytokine profiles, distinguishing it from endogenous hepcidin upregulation via inflammatory pathways.
