Cefamandole is a second-generation cephalosporin antibiotic with broad-spectrum bactericidal activity, primarily through inhibition of bacterial cell wall synthesis via binding to penicillin-binding proteins (PBPs). It exhibits potent in vitro activity against many Gram-positive cocci, including Staphylococcus aureus (MIC90 ~0.5–2 µg/mL) and Streptococcus pneumoniae (MIC90 ~0.12–1 µg/mL), as well as select Gram-negative bacilli such as Escherichia coli, Klebsiella pneumoniae, and Haemophilus influenzae (typical MIC90 range: 0.5–4 µg/mL).

Clinically, cefamandole has been used for respiratory tract, skin, and urinary infections, but its use has declined due to the availability of newer agents and its potential to cause hypoprothrombinemia and disulfiram-like reactions due to the MTT side chain.
