Ulacamten, also known as CK-4021586 or CK-586, is a potent, selective and oral cardiac myosin inhibitor,. CK-4021586 functions by increasing the transition rate of myosin into a force-generating state without affecting calcium transients, thus improving cardiac efficiency. In biochemical assays, CK-4021586 demonstrated sub-micromolar potency, with an EC₅₀ around 0.3 µM for ATPase activity enhancement in purified cardiac myofibrils. In preclinical in vivo models (e.g., rat and dog), the compound increased left ventricular systolic function without increasing myocardial oxygen consumption. Importantly, it showed a high degree of selectivity for cardiac over skeletal and smooth muscle myosin, minimizing off-target effects.
