BMS-986308 is a small-molecule inhibitor of the Renal Outer Medullary Potassium (ROMK) channel, also known as Kir1.1 or KCNJ1, which plays a key role in potassium recycling in the thick ascending limb of the nephron and in potassium secretion in the collecting duct. In preclinical studies, BMS-986308 exhibited potent inhibition of ROMK with an IC₅₀ in the low nanomolar range (~20–40 nM in human ROMK-expressing cell lines), leading to increased urinary sodium and decreased potassium excretion. This pharmacodynamic profile supports its potential as a novel antihypertensive agent, particularly in salt-sensitive hypertension or conditions like hyperaldosteronism.
