Opevesostat (also known as MK-5684 or ODM-208) is a CYP11A1 inhibitor. it blocks the enzyme that plays a crucial role in the production of steroid hormones, including testosterone. Opevesostat suppresses the synthesis of all downstream steroid hormones, including androgens, estrogens, glucocorticoids, and mineralocorticoids. It has shown potent bioactivity in preclinical models of castration-resistant prostate cancer (CRPC), particularly in tumors resistant to androgen receptor (AR)-targeted therapies. Opevesostat is thought to be particularly effective in prostate cancer cells that have developed mutations in the androgen receptor ligand-binding domain (AR-LBD), which can make them resistant to traditional hormone therapies.
