Icanbelimod (CBP-307, YUN88562), is a selective, potent sphingosine-1-phosphate receptor modulator, particularly targeting S1P1 and S1P5 receptors. It acts as an agonist, promoting internalization of S1P1 to inhibit lymphocyte egress from lymphoid tissues. In in vitro assays, CBP-307 exhibits nanomolar potency with EC₅₀ values of approximately 2.8 nM for human S1P1 and 7.3 nM for S1P5, while showing >1000-fold selectivity over S1P2–S1P4. In preclinical models of multiple sclerosis (e.g., EAE mouse models), oral administration of CBP-307 significantly reduced disease severity by decreasing peripheral lymphocyte counts and inflammatory infiltration in CNS tissues.
